US Marriage certificate for passport renewal

Hi,

I am Indian citizen on H1 visa married to US born citizen. We had our marriage registered with Minnesota state. I need to renew my Indian passport. I have changed my name after marriage so i need to fill in the Change of Name form and it is asking for what is the Marriage act\Customs\Rights we are married under. Does anyone has any idea what to fill in the column?

Also my jurisdiction is Chicago where i would be sending my passport document for renewal.
