Need help applying minor's passport!

Hi,

I am applying for my newborn's (in US) Indian passport. Who should sign the documents (except the main application form). Can I just put her name there instead or has it to be my signature? Or should it be just her thumb impression.

Also, do I need to take her with me while submitting the application, I am opting to do that in walkin instead of mailing in the application.
