OCI application for lost PIO card

I just realized that we have lost the PIO card of our minor son. I'm in the process of applying OCI for my son in the same jurisdiction where the PIO was issues (SFO). I don't have a photocopy of the lost PIO card either. Has anyone faced similar situation and if yes, how did you proceed? I'm suspecting we might have lost the PIO card during our last India trip in 2015..
