OCI Name difference affidavit

Applying for an OCI in lieu of PIO for my son. Application was rejected yesterday.
Reason was that the PIO card has my middle name missing.
They want a court order or affidavit explaining the name difference/change.

Can't really get a court order since it's not a name change.

Anybody know the format for a name difference affidavit?

I am not 100% sure, but you can approach any attorney for an affidavit to say the person with full name and the partial name are the same. This just a legal hurdle. The PIO / OCI should match the passport. Could you just apply for a new OCI? If yes, try that.
