PIO permanent address??

hi,
Im applying PIO card for my daughter.Im just confused abt 2 things..

i) what do i enter for permanent address..if its the India address then both mine n my husband's address are not same as the one in our passport.

2) What do I do with the 14th question.It sounds same as the 13th....

dont worry about the address on passport whichever is more current and applicable use that address.

Can you mention what questions 13 and 14 are i dont remember off hand and dont want to pull up an application form

Hi,
Im applying PIO for my daughter....
Somebody pls help me here....for permanent address shud i give Our current US Present address ( which is the same as in my daughter's passport) or shud i give our address at India...

another question is in the 14th question in the form...

Whether the applicant of either of his/her parents or any of his/her grandparents or great grandparents was born and was
permanent resident in territories falling in present day India, Pakistan and Bangladesh, as on the date of the notification of the
Scheme, i.e. 30.3.1999, if so, the following details may be indicated:
