I am applying for PIO card for my children (minors, US born) and found the following information at the CGI houston website :
He / She should submit the following documents:
(a) Application form duly filled in (in duplicate) with four passport size photographs.
(b) A notarised copy each of his/her birth certificate , his/her parent's latest Indian passports (either expired or not yet expired ) and his/her current US/Foreign passport.
(c) The prescribed fee by cashiers check/money order drawn in favour of Consulate General of India, Houston.
(d) Return mailing charges of $ 20/-, if applying by mail.
Note : Please also mention your telephone number in the application form.
My question is about parent's latest Indian passports. My husband and I have applied for new passports as our original ones have expired. The new passports do not bear our current US visa though. Is this OK?
Also should we submit all pages of our passports?
What about proof of residency?? The immihelp website mentions this but not the CGI houston site.. Should I include a utility bill as proof or not?
How do I answer Q13? Whether the applicantor either of his parents or any of his grandparents was born and permanently resident of India at any time ?? Whose details should we provide here?? the child's or mom's or dad's??
Any direction for these questions is much appreciated.
