Documents for PIO card?

Hi, I am appying for a PIO card for my son in New York. Can any one advise, what exactly we needed to submit for PIO with applications. Do we need to send any original documents? Q14 in the application is very confusing, what need to be filled & what are the supporting documents it?

Not sure of NY CG. I went to SFO CG to apply for my son's PIO card. For Q14, I provided the address as mentioned in my passport. As documentary evidence, I took a printout of my and my wife's Demat A/c address- which was the same as my passport address.

However, at SFO CG, the security officer ( a very helpful female) screens your documents, staples them and the hands it you for submission. And she just discarded both my Indian and US address proofs.
