I am trying to fill in part A of OCI process

I was born in India and then I was recently naturalised into US citizenship and got US passport.
For the following questions

Nationality* -- > Should I PUT INDIA or USA ?
Passport No* --> INDIAN PASSPORT OR USA ?
Place of Issue*
