    San Fransico Consulate: Returned PIO Application Need Help

    I had sent the PIO Card application to SF office. Sent 2 forms..with thumb impression. 4 Photos..Our Notorised copy of Passport..Baby's Birth Certificate...and $205

    However..we soon got all the above documents returned from the consulate ....the reason is: "Atleast one parent should sign the form" I do not understand which form and where they had mention this ?

    Am i the only case ? Or anyone else has more information. Please help !!!

    The PIO application form with thumb impressions. For minors the parent needs to sign the form as well.

      Thanks..However they should mention this at the consulate website.
