    Hi,
    I am applying for PIO for my 2 month old child. I have some questions regarding the filling of application.
    1. Do I need to send 5 or 4 passport size photo for my baby ?
    2. Do I need to send notorized copy of her birth certificate or just a photocopy will suffice?
    3. Do I need to send 2 separate proof of current US residence?
    4. The address on passport is not any more valid for myself and my wife. Is it okay to put my father's address as verifiable indian address?
    5. Will the notorized copy of page 2 of my baby's US visa be sufficient since other pages doesn't have any relevant information?


    any guideline fr these will be appreciated

    1. 5 photos. Two should be pasted in box on the application forms. 3 stapled with one of the forms. Size is 35mm*35mm. Different from US Passport Size.

    2. I submitted my documents today at Chicago CG, and he insisted on notarized copies.

    3. One proof is enough.

    4. Yeah. Is any inquiry goes there, they must confirm the details.

    5. it might not have any relevant info, but they need those pages. Its the procedure. I saw a guy was being told about that.
