    Hi,

    I am going to apply for a PIO card for my son and wanted to know how to fill the name section in the form. He has a first, middle and surname but the application has entry only for first name and surname. So where do I put his middle name. I have noticed the same thing on my Indian Passport. We have a Given Name and Surname section but no middle name section. It would be great if someone can advice me on this.

    Where are you filling the PIO form at?

    a copy that I just saw has the space for middle name.

      Huh ?!

      The problem is that PIO is applied and processed directly at any consulate, so there is no standardization of the forms and the intelligent or super intelligent CGI personnel at SF missed it.

      Well then, unless you're far too serious that you have to have the baby's middle name, do ignore and continue.

      An alternate but risky idea would be to make a manual entry of middle name in between the first and the last. However, I would not advice so.

      Finally, you may want to write a separate letter mentioning the same with explanation but not too lengthy and pray that someone cares about you, but then .... sorry you're dealing with different kind of beings here.

      Of course, if you have the time and energy and can wait, then send a P&C letter to the consul general in SF and explain that the form be changed so you can apply properly.

      I saw the forms for some other CGIs in the US and they had the middle initial.

        Things are not that complicated.

        In first name column, write First Name followed by Middle Name.

        Lets assume that someone's full name is Rahul Ramesh Shah.
        Here,
        First Name = Rahul
        Middle Name (Or actually father's name in Gujarat, Maharashtra or husband's name in case of married woman) = Ramesh
        Last Name = Shah

        In that case, you will see that in Indian passport, they will put
        Given name = Rahul Ramesh
        Surname = Shah

        You should follow the same naming convention. I have personally seen people following that convention and getting the PIO card without any problems.
