Applied for PIO at NY on 5th Sep and recieved the card on 12th. Pretty fast and very impressive.

I applied for PIO for me.

Sent:
Appliction form
4 passport photos + 1 pasted on the application form
Notarized copy of my US passport
Notarized copy of my Indian passport
Notarized copy of my parents Indian passport
PA Drivers lic (proof of current address)
Money order + 20$ for express mail return (one MO is ok for both the amounts)
Drivers lic issued in India (only to prove ties to india)

I had some confusion whether I should be sending the original passport (US and Indian) along with the application but you dont have to when applying by mail.

Hi,
I am going to apply PIO for my daughter.
Your message was helpful for the list of documents.

Apply by mail-Do we need to
1.Send all the Indian Citizen Parent's Passport pages with Notarized
2.US citizen passport - All pages with Notarized

I made a copy of all the pages in the passports (indian + us) and got only the main page notarized for each of the passports. Didnt have to notarize every single page.
