PIO Sample Form

Hi All,
Could anyone share with me the sample PIO form that you have filled and applied to US consulate in san Francisco?

Also is it advisable to send the form in postal mail? or should we need to go in person to apply for PIO card?

Any help on this is greatly appreciated.
