PIO Card

Dear Sir/Madmam

I am planning to apply for the PIO card for India. I was born and grew up in India. In 1971 I immigrated to USA and adopted the US citizenship in 1978. I lost my Indian passport some time ago. How can I apply for PIO card in lieu of indian passport.
