Queries for applying PIO card

Hi,

I need to apply PIO card for my newborn son, i have followoing queries. i will appreciate if you can help me.

1) do i need to print form back to back ?
2) The online pdf is acting funny. the fields are getting copied by itself. can i print out and fill it by hand.
3)do i need to put my son thumb impression below the picture?
4) for males which hand thumb is required

---> You may have an older PDF reader, try newer version or use another computer. In worst case scenario, yes you can fill it in by hand, but remember to ensure neat and clean writing. One mistake here and you will be trapped for a long, long painful times.
