PIO - Original passports required to submit?

Hello Everyone,

I'm planning to apply PIO cards for my son, could you please help me answer these questions:
1) Do I need to send all our original passports?
2) List of documents that I need to submit for processing by mail.
3) Application form in SFO consulate website is different from the one available to fill online here (immihelp) which one should I use?

However, as the SF site specifically asks for them, you should probably do as they ask. There seems to be a lot of inconsistency between consulate requirements for the PIO card...

3) Application form in SFO consulate website is different from the one available to fill online here (immihelp) which one should I use?

I'd use the one on the consulate website.

Thanks a lot! for your explanation. Could you please tell me what does "Application in duplicate " means? Do I need to submit two applications, one with original and the other with copy?

I sent two copies of the form, both with original signatures, not copies.
