PIO Extension

Hi,

My wife went to india, and by 19th of October 2009 six months will be past (180 Days). My daughter (1 year old) is holding PIO card, and now my wife wants to stay for 1 more month. We are planning to extend the PIO for my daughter, can any one please let me know the procedure to extend the PIO (I mean where to report) and is there any form to fill up before going to the police station etc. Please let me know the procedure in detail.
