POI Card application

Hi ,

I want to apply for PIO card for my daughter (born in US) ,while filling up the application (form 116) ,the item number 14 needs to be filled up if I have completed item 13 ..

The reson bcs if I need to fill up the item 14 I can give her grand parents details, However I dunno what is the proof I can submit (if passport) I can only submit the xerox copy of that ,I dont think any one will notorize those without original from US , is there any alternative any one can sugest please.

Really appreciate any help on this.

as far as i understood the application form ; if the application is a minor questions 13 and 14 are not applicable to minors
