Damged Passpot Question

Hello,

The lamination on LAST page of my valid passport, which was issued in
New York, 2003 has come out. The front page which has the photo, Date
of Birth, as well as the O1 visa stamp are in good condition .

Is it advicable to apply for a new one? If the Consulate issues a new
one, is the damaged passport returned for using the valid VISAs in it for
entering US?

My passport got wet in washing machine

Hi due to my negligence I washed my passport in a washing machine for nearly 5 minutes, everything looks "ok" nothing has torn apart except a seal on my I-94 has become light and the water print on my passport pages has disappeared. Should I still need to apply for a new passport or is it fine for me to travel with the same passport and get a replacement when I am back in India. The 1st Page, Last page and the Visa are readable.

hii im from austrailia my passport is damaged due to rain drops
can u pls tell me what document are require to apply new passport and where to apply and how much is the fee
and do i require birth certificate as my year of birth is 1986
