I’m preparing an application for a PIO card for my 21 month old son born in New Jersey, USA and holding a USA passport. We both parents are Indian citizens holding Indian passports.
In this context and assuming I’m going to apply by mail, I have few questions and would greatly appreciate your help with these:
1. SIGNATURE: Apart from applicant’s left thumb impressions where signature is needed on the application form, do the parents also have to sign the form?
2. NOTARIZED DOCUMENTS: Indian Consulate NY website states that when sending application by mail, only notarized copies of the documents that are required to be submitted with the application may be enclosed. Does this apply to applicant’s passport as well? Can we submit a notarized copy of the applicant’s passport instead of original?
3. PASSPORT: When submitting notarized passport copies, do we have to submit all the pages in passport or just first three and last three pages? Does this apply to all passport copies – Applicant and Parents?
So, will submitting the following documents suffice for the application of PIO card for my US born infant son?:
a. Application form (filled in duplicate) with left hand thumb impression of the applicant where signature needed
b. Notarized copy of Child's US Passport [First 3 and Last 3 pages only]
c. Notarized copy of applicant’s US birth certificate [First 3 and Last 3 pages only]
d. Notarized copy of parents Indian passports – current and previous [First 3 and Last 3 pages only]
e. Relevant application and return mail fees
Please let me know if I’m missing any other documents that have to be submitted. Your help is greatly appreciated!
