OCI inquiry form - meaning of the status entries

Hi,

I think I understand the general OCI procedure. But what's confusing to me is *where* the steps listed in the OCI Inquiry form actually take place. Could someone explain this?

Registration Status: Granted
Granted by whom? A CGI in US or the MHA in India?
Photo/Signature: Scanned
Scanned by whom? CGI US or MHA India?
Documents Printing Status
Are these the actual OCI documents? Where are they printed? Locally or in India?
Documents Dispatched to Concerned Mission/Office on XX-ZZZ- YYYY
Dispatched by whom? To where?
