filling part b oci misc renewal

In the process of filling part b of misc service for oci renewal (new passport for my minor son and I crossed 50 )
If anybody has filled in the part b I need help.
the format is changed since I filled it in for my daughter last year
Question 2.
Have you surrendered / got your previous overseas citizenship of India card cancelled
Also asks for details

In my sons case I don't remember any details since I had surrendered his old oci card long back more than 5 years ago

Also in my case I just have the present oci card as I have never renewed it earlier

Have you surrendered / got your previous overseas citizenship of India card cancelled = No (if you never canceled OCI). I wonder why this is a question!
