PIO Card-Name change

Hi I changed my last name to my husband's last name during my citizenship process. We have been married for six years but I did not change my name during marriage. Now when I apply for pio card, they sent me my application back saying that I need to send sworn in affadavit fiiled our form to them but when I called them they say they do not know where can i get form from?
any suggestions?
