PIO New York requirements

I got US passport last week and applying for PIO card and have following questions:
1) Do I need to send original US passport or notarized is fine or I need to send both original and notarized. Do I need to notarize all pages?
2) Do I need to 1 application form or 2 copies of application form?
3) Do I need to send original Indian passport or notarized copies?
4) Do US driving license is enough or need to send some other residential proof.

Original plus photocopy - copy of the main page inside the cover is enough. If you're sending in the original with the copy, notarization is not needed.

two copies, both with original signatures.

Original plus photocopy.

4) Do US driving license is enough or need to send some other residential proof.

The web site is not very specific, but you might include copies of lease agreement, utility (electricity, broadband, etc) bill copies for additional confirmation.

Note that a copy of the Surrender Certificate is required, as detailed here.

If you want to have the docs notarized anyway, your bank will probably do it at no charge.

2) For Surrender Certificate, do I need to sign at both places(declarant) and notarized both pages or 2nd declarant is notary

I sent in four total (to Houston): one pasted to each of the two forms, and two in the envelope the photos came in.
