PIO Card sent without surrender certificate

I sent the pio card application and passport/surrender application together with 2 money orders of $385 and $195. I got PIO card and passport back with "passport cancelled" stamped on 2nd page and 2nd last page of Indian passport but no surrender certificate. I have 3 questions:
1) Is passport cancellation stamped is enough or I will receive the surrender certificate in separate mail or what happened to $195 money order?
2) Do I need to send money order of $40 for surrender certificate?
3) Did anyone received the surrender certificate in last 2-3 weeks from NY consulate?

PIO and surrender certificate applications must have been sent separately. PIO dept will not process SC. Meanwhile check if ur $195 has been encashed, if not go for a stop payment. New fee for SC is $20 i guess.
