Need help on OCI application

Hi,
I am trying to apply OCI for my kid who's born in US and a minor. Me and my wife are both Indian citizens. I was able to complete the application online and printed it out.
So in the instructions here it says "Applications should contain BOTH PARENTS signature on the PART B ONLY AND DULY NOTARIZED".

I have following questions:
1) Where should parents sign the application? There doesn't seems to be a designated space for it.
2) For minor does kid have to sign both "Declaration" and "Application for Minor Child" ?

The whole process seems very confusing and with offices closed I am not getting any help from CKGS.

Thanks a lot. The one I got auto-generated doesn't have the fields but I guess I can sign it in that empty space at end.

Both parents can sign, write your names under the signature.
Make sure you do this in front of the notary.

cramn R78 Did we get final answer on where parents need to sign in Page 3 of OCI application form?
Whether minor child has to do thumb impression on both the places? and Parents need to sign on the empty space?
