USA Address on Passport Renewal

Hello Everyone,

I am planning to get my passport renewed in the next couple of weeks. I have a couple of questions:
1. For getting the US Address Printed do I select change of address option ? (Currently my passport has Indian Address that is still valid)
2. What document I need to submit for adding spouse's name on Passport?
