Affidavit in lieu of originals for OCI-VFC

Hi All
Did anyone find the link to download affidavit in lieu of originals when you apply through VFC Global, I did login in VFC global and completed application linking my Government Online application web reference number and paid the fees online using my credit card. And when I said submit, All I see is download labels and checklist (which doesn't ask anything related to minors parents) and nothing else.
