Questions about uploading documents for a New Application:
Current Passport:
1. Do I upload just the information page and include the endorsement/amendment pages in the mail packet or do I upload the endorsement/amendment pages as well?
2. It also says Citizenship Certificate. I'm assuming that means I must upload my Naturalization Certificate. The guide earlier says "If a naturalization certificate is not available, then a Certified True Copy from the local USCIS office must be provided." But, I have mine original naturalization certificate. So will a regular scan and photocopy be enough?
Indian Origin Proof:
3. The upload instructions say "Cancelled Indian passport (first and last page) with stamp showing ‘Cancelled on acquiring foreign citizenship’ as one file." But, earlier, the guide says "Copy of first three and last two pages of the last-held Indian passport of the applicant." So, do I upload the first and last page, but mail a photocopy of the first three and last two pages?
4. Do I need to upload a copy of the renunciation certificate?
5. Is the VOTER CARD required if I have the Indian passport?

6. At the top of the guide it says "Please ensure that all data entries provided in your application match your existing passport. If not, please provide the supporting documentation." Many of my names on my documents have alternate spellings (e.g. some have suffix of -bhai on my middle name, others don't). When I applied for renunciation and a 10 year tourist visa to India, they had me make and notarize a Name Affidavit listing all the spellings and swearing they are one and the same person. Should I include another affidavit and/or upload it? Which document category should I upload it in?
7. Proof of Address does not need to be uploaded?
8. Would there be any harm in uploading and/or sending photocopies of additional documentation?
