New OCI minor - NY area - Questions on OCI online form after printing from govt. site

  • New OCI minor - NY area - Questions on OCI online form after printing from govt. site

    A few questions regarding OCI online form after printing from govt. site:

    1) The OCI checklist from the VFS site says that for the Govt Appl. Online Form, in case of minor, both parents
    signatures are required. But, on the printed form, there is place for only one signature, where can the other parent
    sign? Would having two signatures next to next to each other be acceptable?

    2) The OCI checklist from the VFS site also says notary is required on the last page of the OCI form. I assume this
    would be the page where the parents would sign (the page says page 3 of 6), as pages 4,5 and 6 are instructions
    pages. Is this true?

    3)With regards to photos, it says two passport size photos are required. The physical photo must be affixed to the
    physical form and the digital photo must be uploaded on the Govt. form online. Now,, I uploaded the digital photo
    online and the printed copy of the form already has the picture. Do I still need to affix the physical photo
    on the printed form? If yes, where? Would it be on the digital photo that appears on the printed form? And what about the
    second photo? Just send it with the other documents? Or just send the two photos separately without affixing to the form?

    4) The thumb impression for my son is also appears under the photo from what I had uploaded online. Do I need to
    get my son thumb print page 3, where it asks for the signature of the applicant?

    5)And in the instructions it says documents must be self attested. How is that done? Just sign every page in each
    document?

    6)And the OCI checklist from the VFS site asks for Applicants name, email, passport no., and mobile, do we need to
    fill that? And filling in with handwriting is fine? Or should it be typed in?

    Any help appreciated.

      1) Yes, both parents can and should sign next to each other
      2) Yes, notary on the page where you sign. Remember to sign in front of the notary.
      3) Just send 2 loose photos with the application, no need to affix anything.
      4) Yes, you need his thumb impression. If he is above 5 years, he must sign/write his name.
      5) No need to self attest, I was able to submit all our applications without any problem.
      6) Hand written is fine, don't waste your time by tidying it, as this is just a checklist.
