• For Place of Issue: Do I need to write USA or US DOS or United States Department of State?

• For Previous Passport Number: Do I need to put the cancelled Indian Passport Number or leave it blank because they are asking for previous US Passport #?

I was going through the OCI application and noted one question. It asks for marriage regsteration date, since my marriage certificate has my wedding date and registeration date as different dates, which date do I use?

I put marriage date and didn't have any problems.
