Here is what you have to do assuming you/your spouse are Indian citizen in US on L1/L2 visa...response inline...

    Hi All.

Please give me info. regarding on my Query.

I am on L1 Visa and my wife has L2-dependent visa.My baby was born in USA.

    1) Which visa i need to apply for India ? PIO Card / OCI Card / Visitor visa ?
-- You apply for PIO Card for your baby. Please check for processing time at Consulate. If you have to travel urgently then you will have to apply for Entry Visa. Entry visa is outsourced to TraVisa..Check the consulate visa requirements on website.
    2) Shall I need to apply for Naturalized certificate and Surrender Certificate for Washington India Embassy ?
No naturalized/surrender certs required for US born baby.
    3) What is the procedure to get India visa for 20 years on basis of my Visa status?

PIO is given for 15 years. Your visa status(L1/H1) does not apply to US born baby. Your/your wife being Indian citizen is enough ( proof your Indian Passport )..if you get US citizenship then you will have to apply for OCI for yourself/spouse and submit the surrender cert.

    4) Give me procedure of applying for PIO Card ?

check on the indian consulate website pertaining to your Jurisdiction based on the US state you reside. follow the procedure in this link: http://immihelp.comNew

Note: every consulate office has different requirements regarding application, please read carefully on the website and follow the above immihelp link. it surely helps..

I recently applied for my son at SF consulate as I live in California. Applying for Minor at SF consulate here is what I did:

    1. application form duly filled, parents signature notarized if sending by mail. Do not forget to put baby's thumb print in specimen and signature column.
1.1 2 color photo of the baby.
    2. US passport -original and 2 copies of the Baby
    3. Birth certificate (of baby) color copies -Two notarized
    4. Both parents Indian passport copied Notarized
    5. Two proof of residence copy notarized - any one parent is enough ( i sent my DL & PGE bill)
    6. Fees -MO or cashier's check
    7. Return envelope - ExpressMail preffered by consulate

    Please do needful asap.

    Hope this helps

    Thanks in Advance
