Passport copy verification from Indian consulate in USA

I would like to know if the below steps are correct for verifying passport copy for kids (12 years old)

A. Send the following to consulate
1. notarized copy of first and last page of my kid
2. notarized copy of first and last page of one of the parent
3. Address proof for one of the parent (will submit notarized utility bill in original)
4. notarized I797 copy of kid and one of the parent
5. just the copy of first and last page of kid's passport
6. money order for $12

I have referred to the following page

I would like to confirm the fees of $12 (since there are 2 pages of the passport that will be attested - 1st page and last page).

Can you please confirm?
