Confusing Checklist for OCI

I have four questions regarding OCI application CHECKLIST:

(i) Do we send applicant's ORIGINAL US passport when we start the process?

(ii) Item 6 - US Naturalization Certificate - my daughter was born in US, so she does not have this. What do I submit?

(iii) Do we have to submit 6 copies of the birth certificate of US born person:
Items 7 and 8 - each ask for 2 photocopies of birth certificate
Item 11 - asks for photocopy of item 8

(iv) What is the total processing time for San Francisco?

Please advise.

2. Her notarised birth certificate
