OCI surname difference between US driving license and passport

Hi
My Passport surname is longer than my surname in all my US address proof documents.
But US address proof documents ( like utility bill) , surname is a part of passport name,
Should I file for change of name affidavit while filing
OCI form?
Please help if you have faced similar situation.

You need to decide on one spelling/form for your surname and have the same corrected across all your identity documents - social security, passport, driver's license etc. Else you will face issues, not just for OCI application but in other areas also.

The OCI process is very particular about name accuracy. When I applied, my IN passport had "surname middlename firstname" while my US passport has "surname firstname middlename". And for that I had to submit an affidavit explaining the difference and affirming that I am the same person.

Don't underestimate Indian bureaucracy's capabilities in complicating things and creating problems for you.

Chose one name and send notarized one and the same person affidavit with your application. I had to do that on my OCI Application because my dad had different name on my Indian Passport and his PP had family last name.
