US Minor OCI renewal in India

Hi,

My daughter is stuck in India with my parents. I am in USA. Her passport is going to expire in May. We have a travel plan for July. I have applied for her US passport renewal. I knew I need to apply for OCI renewal also.
Does anyone know the process? how to send a Parental notarized form? Should I mail from the USA to India? What is the address for the OCI application?
