OCI Card in lieu of PIO card for divorced parents

My children have PIO card, I am applying for OCI card now in lieu of PIO. What all documents do I need to submit ? Do I have to submit a copy of the Visas of parents ?? I am divorced and my children live with me (100% physical custody) and shared joint legal custody. For the parent authorization form both parents have to get that notarized or only my signature is suffiicient with notarization? Please let me know

For obtaining the visa to India, do we need to get both parents' signature / Documents any idea ?
