OCI Card

I would like to know if the OCI is issued in the form of a little card or is it a sticker applied on the Canadian Passport?

If it is a sticker applied on the passport, then what happens when the Canadian passport expires? Who has the authority to change the sticker to new passport? Is it the Canadian passport office or the Consulate General of India and how much does it cost to change the sticker?

1. Both.
2. Whether you have to get it transferred to the new passport or not depends upon how old you are. Indian consulate has to do it.
