Question for OCI application

Dear Sir / Madam,
I am eligible and want to apply for OCI card.
I have US Passport. And I had applied for “Renunciation certificate/ surrender last Indian passport” I think I will receive within few days.
I already filled online Part-A form for OCI application on 6th Oct’2010…but soon after cgisf was temporarily not accepting OCI application.
Now My questions:
[1] Should I fill Online PART A again?
[2] In PART-A form XIX…. #4 Place, of birth :I wrote name of town (NEW DEESA) of INDIA. Is it ok or I should write INDIA?
[3] I need to provide one of the followings with application of OCI:

Please provide one:
Two Colour legible photocopies (original may be asked for, if required) of Green Card
or
Two Colour legible photocopies (original may be asked for, if required) of EAD
or
Original Indian passport on which first time entered USA
or
Original I-130 and supporting documents provided at the time of obtaining immigrant visa to enter the USA (see www.uscis.gov for details of obtaining)
or
If applicant is US/ Foreign born, pl. provide two colour photocopies of the birth certificate (In this case, pl. provide documents as per OPTION B at S. No. 8 below)


Should I submit Original Indian passport with cancelled seal on all pages which I will received with “Renunciation certificate/ surrender last Indian passport” ?
I have a copy of Green card, but original was taken back, when I received US citizen certificate.
