OCI Collection Documents to Chicago

Hello,

Have a question around OCI collection documents, when the time comes to send the documents after OCI approval. In addition to US passport, should I send ORIGINAL cancelled Indian passport along with ORIGINAL Surrender certificate to Chicago consulate?
