Help on reninciation application procedure

I had couple questions on the renunciatation application process.

1. Do spouse and self need to send in two separate applications?

2. What should be the amount for the prepaid shipping label that needs to be enclosed?

2. Enough postage so that they can send it back. Go to FedEx or UPS and ask them.

But the indian consulate does not tell me what will be the weight of th epackage the will send back. dont we need to tell UPS the wieght to find the price?

Another question. While filling the Order form for the renuncuation of citiziindian citizenship on travisa website, there are three sections of questions. When done it asks us to review all our answers. In the review section I found a totally new question " Can you provide your most recent indian passport?" and it was answered as "No". I tried to click the Edit button to go back and see that question to make it YES. But there is no such question!!

Either I am doing something wrong or the website is stupid. I checked few times.
Any suggestions guys? The consualate does not pick up the phone or respond to email request.
