What are the documents needed for OCI card for Canadian citizens living in US

Hi,

I'm a canadian citizen living in the US. Need to apply for OCI card. I only see requirements for US citizens in Travisa website. As I need to apply for the OCI in US, need to know what the requirements are. My many calls to Travisa for information did not yield any useful answers. They themselves do not seem to know the answers as I keep getting conflicting information at different times from the same folks.
