Question Passport Renewal form

In the current form they want me to fill online there are two different addresses
1. Address to be Printed in Passport
2. Other Address

and both are mandatory. Also they ask proof of address in US.

I am confused which address is which. Can somebody who has similar experience or know exact answer help me please?

Do I need to give my US address in 1 or 2?
Do I need to mention my Indian address at all ? If yes does it go in 1 or 2? And do I need to provide proof for that?
