But, if something happened and I was ever in the position to need care at this Kendallville hospital ( when visiting my mom or…) I could still use it but pay the $250 non PPO fee ….and still be covered by the policy?

If you go out of network there will not be any negotiated rate.

Meaning that I pay my deductible and IMG pays the rest. ? For instance.. if I had to go to the emergency room at Parkview Noble… I pay my deductible and IMG picks up the rest.. even though Parkview Noble is not a PPO ??

I am in Haiti … and visit my mother in Kendallville, Indiana… What if I am in an auto accident or have a heart attack… or something that required me to go to Kendallville Noble, which is 1 miles away from my mom’s house…. The bottom line is… am I covered ? or do I have to try to get to Lagrange, 25 miles away ?

You can always go out of the network.

If you go out of the network, you have to first pay the deductible. Then insurance company pays 80% for first $5,000 in covered medical expenses, and 100% after that up to the policy maximum.
            Thanks & Regards
