Hi,
I am considering to visit USA, for Elective in a Hospital in New york. Following is their insurance requirement.
I would appreciate to advise below coverage process and cost of insurance. Period of my visit is May 25, 2017 to June 25, 2017. My date of birth is 05.08.1993.

Minimum Required Coverage:
1. $300,000 Inpatient Services: daily hospital room & board, intensive care unit, inpatient and outpatient operating/treatment/recovery rooms, medical services and supplies such as medications provided while inpatient, physician visits, imaging, emergency treatment of injury even if not admitted, professional fees of doctors, surgeons and nurses.
2. $100,000 Outpatient Services: professional fees of doctors, office visits, referrals, X-rays, labs, emergency room fees (illness and injury), mental disorder & substance abuse,
3. Ambulance Services Expenses: Ground ambulance service to a hospital for necessary emergency care.
4. Prescription Drugs: for both inpatient and outpatient.
