Passport for Minor Child

i need to apply for a new passport for my child - 2 month old

i got the Affidavit signed by Indian consulate Chicago

i am in US now

do i need to add my name in my wife's passport before submitting the application form for the child (some agents and other people say it is must)

I would like to apply for sworn affidavit at Indian Embassy, Washington. My kid is in INDIA with my wife. Could you pls explain me what is the process to get sworn affidavit.
