Help needed: H1B into Greencard (timing)

My second term H1B ends October 2006 and I (and my employer) want to start the Greencard process.

I have heard that:
1. I have to start the Greencard process 1 year before the end of the second H1B term?;
2. That the term starts at the first entry under the first term of H1B, not at the issue date?
3. that it is possible to get an extension for 1 more year under H1B?

Could anybody confirm or deny the above or provide extra advice?

my Friends did got 1 year extensiion. He applied 1 year before his 6 year expired. As long as you apply 1 year before your 6 year exipration, you can get extension... one of my friend even got 1 year extension for 2 times....
