Some helps about obtaining the Optional Practical Training (OPT)?

Hello guys, i just graduated from my university and now am trying to get my OPT to be eligible to work as an F-1 student. My big concern is that i have to write a letter addressed to the DSO requesting OPT, including:

1) A description of the practical training being sought
2) How the proposed practical training relates to the field of study
3) Why practical training is not available in the student's country
4) Start and dates of requested employment

This is it. so if i can get a sample to answer those questions, it will help me a lot.

2. Write about the relation of above with your field
3. Heard this for the first time. Just write a small paragraph. blah blah it.
4. This is up to you. You can start as late as 60 days after last day of classes. Once this is fixed, you cannot change it.
