Hi,
Say, my wife's maiden name is Trina Bhattacharya (current passport). After marriage, her name is Trina Kar (old passport).
How do I fill up the the following two questions under the section "Spouse's Full Name (include Maiden Name)"
Spouse's Surnames:
Spouse's Given Names:
