POA form seems to be for the Indian citizens at NY but OK at Washington DC

Hi,

At the end of the POA form at NY ( in DECLARATION) it ask if I have "not" surrendered my Indian Passport.

The same form at the DC clearly mentions that that section is for Indian citizens only.

I am US citizen of Indian origin (OCI) and wonder if I could use that NY form as is.

I just spoke with consulate lady and she says to strike that part out and use the CGNY form as is.
